Please fax or post the completed form and supporting documentation to the RTT Medical Credit Risk Management Department.

Kindly include a copy of the Licence Certificate of the Department of Health (New Pharmacies and Dispensing Doctors) and Pharmacy Council Of SA Registration Certificate if applicable. In addition, please remember to state that you wish to be set up to place orders via PHDOnline.
